Abstract

Objective: To determine perceptions of smile esthetics and related factors among adults aged 18-30 in Hanoi, 2025. Subject and methods: This was a cross-sectional descriptive survey using a 0-10 visual analogue scale (VAS) to evaluate selected smile images among 535 students including 259 first- and second-year students and 276 fifth- and sixth-year students trained in smile and facial esthetics at the University of Medicine and Pharmacy-Vietnam National University in 2025. Results: In group 1, sex differences were mainly observed in G3-G4, H2-H3 and D3 whereas group 2 showed broader and more pronounced sex-related differences, particularly in G1, G3-G4, M2-M3 and D1-D3. Group 1 scored significantly higher than group 2 for G1-G4 (p<0,0001). Within groups the ranking was G5=G1>G2>G3>G4 for group 1 and G5>G1=G2>G3>G4 for group 2 and the overall sample. Across M1-M3, group 1 consistently showed higher scores than group 2 (p < 0,001) with a clear decreasing trend from M1 to M3 within each group. Conclusions: Smile esthetic perceptions among adults aged 18-30 are influenced by sex, age, and aesthetic knowledge and experience with females showing stricter esthetic standards than males. Gummy smile level, gingival margin symmetry and maxillary central incisor crown height, midline deviation, and maxillary midline diastema are directly associated with and strongly influence smile esthetic perceptions in young adults.
