Abstract

Objective: To evaluate the outcomes of obstetric management in pregnant women with systemic lupus erythematosus (SLE) during the third trimester of pregnancy at Bach Mai Hospital. Methods: A retrospective descriptive study was conducted on 45 medical records of pregnant women with SLE whose pregnancies ended in the third trimester at Bach Mai Hospital between January 1, 2024, and December 31, 2024. Results: The mean age of the study population was 29.0 ± 4.8 years. The average disease duration was 4.3 ± 2.5 years. SLE was newly diagnosed during pregnancy in 15.6% of the patients. Combination therapy with methylprednisolone and hydroxychloroquine (HCQ) was administered in 17.8% of cases. The rates of term delivery, preterm delivery, stillbirth, and intrauterine growth restriction were 51.1%, 44.4%, 2.2%, and 45.5%, respectively. The cesarean section rate was 95.5%, predominantly elective procedures. Maternal complications during pregnancy and the postpartum period occurred in 15.4% of patients, mainly in those with a gestational age of less than 37 weeks. Conclusion: The rates of adverse pregnancy outcomes, such as preterm birth and intrauterine growth restriction, remain high. Enhanced patient education and close antenatal monitoring are essential, along with strengthened multidisciplinary collaboration in the management of pregnancy in women with SLE
