ASSOCIATION BETWEEN CERVICAL LENGTH AND PREGNANCY OUTCOMES IN WOMEN WITH THREATENED PRETERM BIRTH: A STUDY AT HANOI OBSTETRICS AND GYNECOLOGY HOSPITAL

Abstract

Objective: To evaluate the association between cervical length and pregnancy outcomes in women with threatened preterm birth and a short cervix (<25 mm) at Hanoi Obstetrics and Gynecology Hospital. Subjects and Methods: A descriptive longitudinal study was conducted on 70 pregnant women diagnosed with threatened preterm birth, with gestational ages ranging from 22 to 34 weeks and a cervical length of less than 25 mm. All participants were admitted and treated at Hanoi Obstetrics and Gynecology Hospital between August 2024 and August 2025. Pregnancy outcomes, including gestational age at delivery, neonatal birth weight, neonatal complications, and maternal complications, were recorded and analyzed. Results: The mean gestational age at delivery was 32.7 ± 3.5 weeks, and the mean neonatal birth weight was 1950.7 ± 657.9 g. Neonatal complications occurred in 70% of cases, with respiratory distress (44.3%) and neonatal infection (28.6%) being the most common. Cervical length was significantly associated with neonatal birth weight (p = 0.02), with the lowest mean birth weight observed in women with a cervical length < 25 mm (1 616 g). An increasing trend in neonatal complications was observed as cervical length decreased, from 33.3% in the 21–25 mm group to 83.3% in the <5 mm group. No statistically significant association was found between cervical length and maternal complications, including postpartum infection and postpartum hemorrhage. Conclusion: A short cervical length (<25 mm) in women with threatened preterm birth is significantly associated with adverse pregnancy outcomes, particularly low neonatal birth weight and increased neonatal morbidity.
