CLINICAL CHARACTERISTICS AND TREATMENT OUTCOMES OF CHRONIC HAND CONTACT DERMATITIS AT CAN THO CITY DERMATOLOGY HOSPITAL IN 2025

Abstract
Introduction: Chronic hand contact dermatitis is a common dermatological condition characterized by a prolonged and persistent course. Treatment effectiveness depends on various factors, including disease duration and environmental living conditions. Therapeutic approaches include elimination of causative factors, topical treatments, with or without the addition of systemic medications. Objectives: To describe the clinical characteristics and evaluate treatment outcomes of chronic hand contact dermatitis. Materials and Methods: A cross-sectional descriptive study was conducted at Can Tho City Hospital of Dermatology in 2025, involving 50 patients diagnosed with chronic hand contact dermatitis. Results: Patients aged over 45 years accounted for the majority (50%). Palmar involvement was the most common lesion site (44%). Among contact-related etiological factors, soap was the most frequent (36%), followed by metals (24%). Regarding treatment modalities, emollients were prescribed for all patients (100%); elimination of causative factors and the use of protective gloves during work were recommended in 90% and 80% of cases, respectively. Topical corticosteroids were used in 84% of patients. Most patients required a treatment duration longer than 4 weeks (60%), while 40% were treated for 4 weeks or less. After treatment, complete response was observed in 56% of patients, partial response in 34%, and no response in 10%. The recurrence rate after more than one month was 84%, whereas recurrence within one month accounted for 16%. Conclusions: The palm was the most frequently affected site, and soap was the most common causative factor. Most patients received combination therapy with topical and systemic medications. Emollients were used in all patients. After 8 weeks of treatment, marked clinical improvement was observed. Local and systemic adverse effects were mild and transient.
