Abstract

Introduction: Primary hyperhidrosis is a condition characterized by excessive sympathetic nervous system activity, significantly affecting quality of life. The hallmark of primary hyperhidrosis is localized sweating in specific regions of the body, symmetrically on both sides. This condition is not life-threatening but greatly diminishes patients’ quality of life; for example, palmar hyperhidrosis impacts work and causes psychological discomfort during social interactions. Endoscopic thoracic sympathectomy (ETS) is an effective and minimally invasive surgical treatment.


Objective: To evaluate the outcomes of ETS in treating primary localized hyperhidrosis at the Department of Surgery – 19-8 Hospital.


Methods: Cross-sectional retrospective study.


Results: A total of 126 patients were included in the study from January 2025 to December 2025. The average age was 28.9 ± 6.4, with 63.5% being female..Complete elimination of hand sweating was achieved in 97.1%, axillary sweating in 100%, and foot sweating reduction by 20.3%. There was one case of mild pleural effusion (0.8%), with no pneumothorax or severe complications. Two patients had a history of right lung adhesion due to tuberculosis, and only the left side was operated on. Quality of life improved in 96.9% of patients; 87.5% were satisfied or very satisfied. The T3–4 ganglionectomy group had the highest satisfaction rate (90.1%).


Conclusion: Thoracoscopic sympathectomy (ETS) is a safe and effective method that significantly improves the quality of life for patients with localized hyperhidrosis, with a low complication rate and stable results.
