ASSESSMENT OF ANGINA STATUS IN PATIENTS WITH CHRONIC CORONARY SYNDROME USING A PATIENT-HELD DIARY

Abstract
Introduction: Angina pectoris is a common symptom in patients with chronic coronary syndrome (CCS) and has a significant impact on quality of life. Systematic monitoring of anginal symptoms using a patient-held diary may support optimization of outpatient management and improve treatment outcomes.
Objectives: (1) To assess angina status in patients with chronic coronary syndrome receiving outpatient treatment at Cho Ray Hospital; (2) To describe changes in anginal characteristics recorded through a patient-held diary during follow-up.
Methods: This retrospective, cross-sectional descriptive study included 86 patients diagnosed with chronic coronary syndrome according to the 2019 ESC guidelines, who were treated at the Cardiology Outpatient Clinic of Cho Ray Hospital and participated in a patient-held angina diary (AAI) from February 2021 to August 2021. Recorded variables included angina frequency, intensity (graded on a 1–6 scale), and triggering conditions (at rest or on exertion).
Results: The mean age of the study population was 67.01 ± 10.58 years; males accounted for 44.2%; Hypertension was present in 100% of patients and diabetes mellitus in 82.6%. At baseline, the mean number of angina episodes was 3.1 ± 1.5 episodes per month. Most episodes were of moderate intensity grade 2/6 (59.3%) and occurred predominantly at rest (68.6%). During follow-up using the angina diary, anginal symptoms showed an improving trend: angina frequency decreased from 2.94 episodes/month to 2.08 episodes/month at the final visit; mean angina intensity decreased from 2.39 to 1.64. The angina trigger score increased, suggesting a relative shift toward exertional angina compared with rest angina.
Conclusion: Angina status in patients with chronic coronary syndrome tended to improve during follow-up. Guiding patients to record symptoms in a patient-held diary enables clinicians to more accurately assess angina severity, thereby supporting optimization of therapeutic strategies.
