STUDY ON HYSTERECTOMY FOR THE TREATMENT OF BENIGN GYNECOLOGICAL DISEASES AT TAM ANH GENERAL HOSPITAL, HANOI

Abstract
Objective: To describe the clinical and paraclinical characteristics and to evaluate the surgical outcomes of hysterectomy performed for benign gynecological diseases at Tam Anh General Hospital, Hanoi.
Materials and Methods: This was a prospective cross-sectional descriptive study conducted on 94 patients who had indications for hysterectomy due to benign gynecological conditions (uterine fibroids, endometriosis/adenomyosis, endometrial polyps, CIN, etc.) from July 2022 to December 2023 at Tam Anh General Hospital, Hanoi. Study variables included age, obstetric history, surgical indications, uterine size, uterine weight, operative time, complications, and length of hospital stay.
Results: The mean age was 44.64 ± 5.8 years (range: 30–59). Women with two previous births accounted for 61.7%, while 10.6% were nulliparous. Uterine size equivalent to a 3–4 month pregnancy was observed in 68.1% of cases. Preoperative anemia status showed: no anemia in 63.8%, mild anemia in 26.6%, moderate anemia in 7.4%, and severe anemia in 2.1%. The main indication for hysterectomy was uterine fibroids (75.5%), followed by endometriosis/adenomyosis (20.2%), CIN III (3.2%), and endometrial polyps (1.1%). Laparoscopic surgery was successfully completed in 88.3% of cases; conversion to laparotomy occurred in 6.4%, while 5.3% underwent primary open surgery. Subtotal hysterectomy accounted for 53.2% and total hysterectomy for 46.8%. Bilateral adnexectomy was performed in 18.1% of patients, while ovarian preservation was achieved in 81.9% (among them, 50.0% did not preserve the fallopian tubes). The mean operative time was 84.69 ± 19.1 minutes (range: 40–130 minutes). The mean uterine weight was 340.96 ± 202 g (range: 50–1198 g). The average hospital stay was 2.97 ± 1.01 days (range: 2–7 days). No severe complications were recorded.
Conclusion: Laparoscopic hysterectomy for benign gynecological diseases at Tam Anh General Hospital, Hanoi, demonstrates a high success rate, rapid postoperative recovery, safety, and effectiveness, consistent with the recommendations of FIGO, ACOG, and RCOG.
