MICROBIOLOGICAL CHARACTERISTICS AND ANTIBIOTIC RESISTANCE OF PATHOGENIC BACTERIA IN SEPSIS PATIENTS AT THE ICU OF TRA VINH GENERAL HOSPITAL

Abstract

Background: Sepsis is a severe infectious condition with high mortality, in which antibiotic selection depends on the bacterial spectrum and resistance pattern in each healthcare setting.


Objectives: To determine the prevalence and antibiotic resistance of bacterial strains causing bloodstream infections in patients treated at the Intensive Care Unit, Tra Vinh General Hospital.


Methods: This retrospective cross – sectional study included 129 patients diagnosed with sepsis between May 2024 and May 2025 at Tra Vinh General Hospital.


Results: The positive blood culture rate was 38.76%. Gram – negative bacteria predominated (60%), with Escherichia coli (16.00%) and Klebsiella pneumoniae (14.00%) being the most common Gram – negative pathogens. Among Gram – positive bacteria, coagulase – negative Staphylococcus (CoNS) accounted for the highest proportion (36.00%). The antimicrobial resistance of the isolated strains was assessed as severe. Escherichia coli and Klebsiella pneumoniae showed very high resistance rates to second- and third-generation Cephalosporins and Fluoroquinolones, but remained susceptible to Carbapenems and Amikacin. Enterobacter spp. exhibited complete resistance to many β – lactam antibiotics and Trimethoprim/sulfamethoxazole. CoNS displayed high resistance to Oxacillin, Erythromycin, and Clindamycin, while remaining sensitive to Vancomycin and Linezolid.


Conclusion: Gram – negative bacteria were identified as the predominant causative agents of sepsis in the Intensive Care Unit. The concerning levels of antibiotic resistance, particularly against Cephalosporins and Fluoroquinolones, highlight the need for careful consideration in empirical therapy and underscore the importance of establishing appropriate antibiotic treatment protocols.
