Abstract

Background: Hypertension (HTN) is a global health priority, with home-based self-management serving as a cornerstone for effective disease control.

Objectives: To assess the level of hypertension self-care practices using the Hypertension Self-Care Profile (HTN-SCP) and to identify associated factors among patients.

Methods: A cross-sectional descriptive study was conducted on 384 patients at the Family Medicine Clinic, Le Van Thinh Hospital, from November 2024 to March 2025. Data were collected using the validated 20-item Vietnamese version of the HTN-SCP scale.

Results: The mean self-care practice score was 53,6 ± 8,37 (out of a maximum of 80 points). According to Bloom’s cut-off points, 55,5% of patients achieved a moderate level of self-care or higher. The highest scores were observed for medication adherence and follow-up visits, while dietary control and reading food labels received the lowest scores.

Conclusion: Hypertension self-care practices among patients were at a moderate level. There is a need for personalized counseling interventions, particularly focusing on male patients, those with lower educational levels, and those residing in rural areas to improve treatment outcomes.
