EVALUATING SLEEP QUALITY AMONG ELDERLY AT TWO CARDIOLOGY OUTPATIENT CLINICS OF THONG NHAT HOSPITAL, HO CHI MINH CITY

Abstract

Background: Sleep disorders and cardiovascular diseases in older adults are increasingly prevalent. Poor sleep quality (SQ) has been associated with various adverse outcomes, including hypertension, heart failure, stroke, and depression. The Pittsburgh Sleep Quality Index (PSQI) is currently one of the most widely used tools for assessing sleep quality. However, in Vietnam, studies on sleep quality among older outpatients with cardiovascular diseases remain limited.


Objectives: To determine the prevalence of poor sleep quality using the PSQI and to investigate factors associated with sleep quality among older outpatients with cardiovascular diseases.


Methods: A descriptive cross-sectional study was conducted among patients aged ≥60 years with cardiovascular diseases receiving outpatient care at the General Cardiology Clinic A and the Interventional Cardiology Clinic A of Thong Nhat Hospital, Ho Chi Minh City. Sleep quality was assessed using the PSQI, with poor sleep quality defined as a PSQI score >5. Univariable and multivariable logistic regression analyses were performed to identify factors associated with poor sleep quality.


Results: Among 412 older outpatients with cardiovascular diseases, the prevalence of poor sleep quality according to the PSQI was 46.6%. Factors associated with poor sleep quality included female sex (OR = 4.46, 95% CI: 2.25–8.82, p < 0.001), lack of regular exercise (OR = 4.77, 95% CI: 2.57–8.85, p < 0.001), chronic heart failure (OR = 3.32, 95% CI: 1.07–10.26, p = 0.037), history of stroke (OR = 3.10, 95% CI: 1.06–9.10, p = 0.039), osteoarthritis (OR = 1.89, 95% CI: 1.01–3.54, p = 0.045), chronic pain (OR = 7.60, 95% CI: 1.81–31.98, p = 0.006), and depression (OR = 14.58, 95% CI: 3.72–57.10, p < 0.001).


Conclusions: The prevalence of poor sleep quality among older outpatients with cardiovascular diseases was relatively high. Factors associated with poor sleep quality included female sex, lack of exercise, chronic heart failure, history of stroke, osteoarthritis, chronic pain, and depression. These findings highlight the need for enhanced sleep assessment in this patient population in routine clinical practice.
