MULTIPARAMETRIC MRI VALUE OF DIFFUSION TENSOR IMAGING, MAGNETIC RESONANCE SPECTROSCOPY, AND PERFUSION IMAGING IN DIFFERENTIATING HIGH-GRADE GLIOMAS FROM SOLITARY BRAIN METASTASIS

Abstract
Objective: To evaluate the efficacy of Diffusion Tensor Imaging (DTI), Magnetic Resonance Spectroscopy (MRS), and Magnetic Resonance Perfusion (MRP) in differentiating High-Grade Gliomas (HGG) from Solitary Brain Metastases (SBM).
Methods: This retrospective study included 41 patients (24 HGG, 17 SBM) who underwent multiparametric MRI prior to intervention. FA, MD, rCBV, rCBF, and metabolic ratios (Cho/NAA, Cho/Cr) were measured in the intratumoral (t) and peritumoral (p) regions. All findings were correlated with histopathology. Diagnostic performance was assessed using ROC curve analysis and multivariate logistic regression.
Results: In the intratumoral region, only MDt (P = 0.011) and Cho/NAAt (P < 0.001) showed significant differences. Conversely, most peritumoral parameters (except FAp) exhibited distinct differences between the two groups. The peritumoral Cho/Crp ratio achieved the highest diagnostic performance (AUC 0.939) and was the sole independent predictor in the multivariate model. At a cutoff of 1.22, Cho/Crp yielded a sensitivity (Se) of 87.5%, specificity (Sp) of 94.2%, and accuracy (AC) of 90.3%.
Conclusion: Multiparametric MRI in the peritumoral region is highly valuable for differentiating HGG from SBM. Cho/Crp is the strongest independent predictor, while rCBVp provides optimal specificity for HGG diagnosis.
