DETERMINING THE RELATIONSHIP BETWEEN MAXILLARY MOLAR ROOTS AND THE MAXILLARY SINUS

Abstract

Objective: To determine the relationship between maxillary molar roots and the maxillary sinus using Cone Beam Computed Tomography (CBCT), and to analyze differences according to root type, tooth position, and side.


Methods: A cross-sectional study was conducted on 80 patients at the Department of Odonto-Stomatology, Hanoi Medical University Hospital. A total of 320 maxillary molars (teeth 16, 17, 26, 27) comprising 960 roots (mesiobuccal, distobuccal, palatal) were evaluated and categorized into three levels: no contact, sinus floor contact, and sinus penetration. Statistical analysis was performed using the Chi-square test (p < 0.05).


Results: Among 960 roots, 530 (55.2%) showed no contact, 160 (16.7%) showed sinus floor contact, and 270 (28.1%) penetrated the sinus. The highest penetration rate was observed in mesiobuccal roots (43.8%), followed by distobuccal roots (34.4%) and palatal roots (6.3%) (χ² = 210; p < 0.001). Teeth 16 and 26 demonstrated higher sinus involvement compared to teeth 17 and 27 (p < 0.05). No significant difference was found between right and left sides.


Conclusion: Mesiobuccal roots present the highest risk of sinus involvement. Preoperative CBCT evaluation is essential to minimize surgical complications.
