Abstract

Background: Synovial chondromatosis is a rare benign condition in children caused by synovial metaplasia. Its histopathological features may overlap with osteochondroma.

Objective: To describe a rare pediatric case of synovial chondromatosis, thereby emphasizing the decisive role of direct intraoperative gross evaluation—integrated with clinical findings, imaging studies, and histopathological features—in achieving an accurate differential diagnosis of ectopic osteochondral lesions.

Method: To describe a case presentation: An 5-year-old girl presented with right lower leg pain without trauma.

Result: Plain radiography incidentally revealed an ectopic bony structure not connected to the tibia or fibula. Surgical excision was performed. Gross intraoperative inspection demonstrated that the osteochondral mass originated from the synovial membrane, without osseous continuity with adjacent bone, which was the key diagnostic feature. Although histopathology suggested an osteochondroma-like lesion, the final diagnosis.

Conclusion: Direct intraoperative identification of synovial origin is crucial for definitive diagnosis of pediatric synovial chondromatosis, even when histopathology suggests osteochondroma.
