INVESTIGATION ON UTEROTONIC USE IN POSTPARTUM HEMORRHAGE PROPHYLAXIS AT DONG NAI - 2 HOSPITAL

Abstract
Background: Postpartum hemorrhage (PPH) is a leading cause of maternal mortality. This condition can be prevented by using uterotonic agents.
Objectives: To describe the patterns of prophylactic uterotonic use, and to identify factors associated with volume of blood loss after delivery.
Materials and methods: Descriptive cross-sectional study was conducted on medical records of parturients receiving uterotonic(s) for PPH prophylaxis at the Obstetrics Department, Dong Nai-2 Hospital (January 1st, 2025 - March 31st, 2025). Data collected included patients’ characteristics and patterns of uterotonic use. Multivariate logistic regression was used to identify factors associated with postpartum blood loss volume, with statistical significance set at p<0.05.
Results: There were 415 medical records included (median age 30 (26-33) years). Parturients having abnormal obstetric history were 16.5%. Oxytocin, ergometrine, carbetocin, and misoprostol were indicated in 100%, 44.1%, 35.7%, and 8.4% parturients, respectively. Nearly half (47.7%) received a combination of two uterotonics. The median estimated blood loss volume was 200 (150-200) ml. Postpartum complications occurred in 4.8% of cases, mostly poor uterine contractions (3.1%). Cesarean delivery and abnormal obstetric history were associated with increased blood loss volum ≥200 ml (p<0.001).
Conclusions: Oxytocin was predominantly indicated for PPH prophylaxis. Closer monitoring is needed in parturients with cesarean delivery or abnormal labor history.
