EVALUATION OF OUTCOMES OF ORBICULARIS OCULI MUSCLE TIGHTENING COMBINED WITH UPPER EYELID SKIN EXCISION AND CREASE FORMATION IN THE TREATMENT OF MODERATE TO SEVERE LATERAL UPPER EYELID DERMATOCHALASIS

Abstract

Objective: This study aimed to evaluate the outcomes of orbicularis oculi muscle tightening combined with upper eyelid skin excision and crease formation in the management of moderate to severe lateral upper eyelid dermatochalasis. The procedures were performed at the Department of Oculoplastic and Facial Aesthetic Surgery, Vietnam National Eye Hospital, from August 2025 to March 2026.


Subjects and Methods: This was an uncontrolled interventional descriptive study conducted on patients diagnosed with moderate to severe lateral upper eyelid dermatochalasis, based on the classification proposed by Jacobs (2014). Sample size: 20 eyes (10 patients).


Results: The male-to-female ratio was 1:4, with a mean age of 56.5 years; 60% of patients were in the 40–60 age group. Moderate dermatochalasis accounted for 80% of cases, while 20% were classified as severe. Dynamic wrinkles were observed in 30% of patients, whereas 70% presented with mixed-type wrinkles. A total of 80% of patients had an upper eyelid crease height of less than 3 mm, while 30% had a crease height ranging from 3 to 6 mm. Postoperatively, 90% of patients achieved successful outcomes. A marked improvement in upper eyelid dermatochalasis, particularly in the lateral region, was observed following surgery.


Conclusion: Orbicularis oculi muscle tightening combined with upper eyelid skin excision and crease formation is an effective surgical approach for upper eyelid rejuvenation, especially in patients with moderate to severe lateral dermatochalasis.
