EVALUATION OF AUTOLOGOUS DERMAL GRAFT IN THE SURGICAL MANAGEMENT OF PEYRONIE'S DISEASE

Abstract
Background: Peyronie's disease (PD) is an acquired fibrotic disorder of the tunica albuginea causing penile curvature and erectile dysfunction (ED). Plaque excision with tissue grafting is the gold standard when curvature exceeds 60°. Autologous dermal graft (ADG) offers low cost, readily available and is well-suited to Vietnamese healthcare settings.
Objectives: To evaluate penile straightening rate, length preservation, erectile function (IIEF-5), and patient satisfaction following ADG surgery for PD.
Methods: Prospective case series of 46 men with stable PD, curvature ≥ 60° and IIEF-5 ≥ 12, who underwent plaque excision and ADG at Binh Dan Hospital from January 2023 to December 2025. Outcomes assessed: straightening success (residual curvature < 15°), penile length, de novo ED, IIEF-5, complications, and satisfaction.
Results: Mean preoperative curvature was 72.3 ± 14.8°. Over mean 24.6-month follow-up: straightening success 87.0%; length preservation 89.1%; de novo ED 15.2% - strongly associated with type 2 diabetes and lower baseline IIEF-5; IIEF-5 at 12 months 16.2 ± 4.7 (p = 0.12); patient satisfaction 84.8%. Wound infection rate was 2.2% (Grade I Clavien-Dindo).
Conclusion: ADG is safe and effective for severe PD curvature while remaining cost-effective for Vietnamese practice.
