EVALUATION OF INFERIOR ALVEOLAR NERVE SENSORY RECOVERY AFTER BILATERAL SAGITTAL SPLIT OSTEOTOMY

Abstract
Objective: To evaluate sensory recovery of the inferior alveolar nerve and identify factors associated with persistent sensory disturbance after bilateral sagittal split osteotomy (BSSO).
Subjects and methods: A prospective longitudinal study with 24-month follow-up was conducted on 94 patients undergoing BSSO, including 47 patients with mandibular advancement and 47 patients with mandibular setback, at Van Hanh General Hospital.
Results: Subjective sensory disturbance at 1 week was observed in 93.6–95.7% of patients in both groups. Levels C and B recovered almost completely between 6 and 24 months, whereas Level A remained abnormal in 6.38–8.51% of patients at 24 months. No statistically significant difference was found between the two groups. VAS showed the strongest correlation with Level A (rho = 0.82–0.90). Surgical movement ≥6.00 mm (OR = 3.49; p = 0.010) and age ≥25 years (OR = 3.11; p = 0.030) were independent factors associated with persistent sensory disturbance at 6 months.
Conclusion: BSSO provides good sensory recovery; however, fine sensory function may remain abnormal after 24 months. Greater surgical movement and older age should be considered during preoperative counseling.
