SURGICAL OUTCOMES OF OPEN REDUCTION AND INTERNAL FIXATION WITH PLATE-AND-SCREW CONSTRUCTS FOR INTERCONDYLAR DISTAL HUMERUS FRACTURES IN ADULTS AT HO CHI MINH CITY HOSPITAL FOR TRAUMA AND ORTHOPAEDICS FROM 2020 TO 2025

Abstract
Objective: To evaluate the outcomes of open reduction and internal fixation with plate-and-screw constructs in the treatment of closed intercondylar distal humerus fractures in adults at the Hospital for Trauma and Orthopaedics during the period 2020–2025.
Materials and Methods: A retrospective descriptive cross-sectional study was conducted on 63 patients aged 18 years or older with closed AO type C intercondylar distal humerus fractures. All patients underwent open reduction and internal fixation using plate-and-screw constructs between January 2020 and December 2025. Evaluated parameters included clinical characteristics, radiographic findings, surgical techniques, fracture union, elbow functional outcomes, and postoperative complications.
Results: Patients older than 60 years accounted for the highest proportion (43%). The predominant mechanism of injury was traffic accidents (63%). AO type C2 fractures were the most common (52%). A triceps-splitting approach was used in 98% of cases, and orthogonal dual plating was applied in 90%. Good reduction quality was achieved in 86% of patients, and the fracture union rate was 94%. Elbow flexion less than 120° was observed in 44% of patients, while 44% had an extension deficit greater than 15°. The rates of infection and secondary displacement were both 8%, and 3% of patients required revision surgery due to implant failure. According to the Mayo Elbow Performance Score (MEPS), 74% of patients achieved fair to excellent functional outcomes.
Conclusion: Open reduction and internal fixation with plate-and-screw constructs for the treatment of closed intercondylar distal humerus fractures in adults resulted in a high union rate, satisfactory restoration of elbow function, and an acceptable complication rate. This technique is an effective treatment option for AO type C intra-articular distal humerus fractures.
