ASSESSMENT OF ADVANCED LIVER FIBROSIS RISK IN OLDER ADULTS WITH TYPE 2 DIABETES MELLITUS AND NON-ALCOHOLIC FATTY LIVER DISEASE

Abstract

Background: Advanced liver fibrosis is a critical clinical issue in patients with type 2 diabetes mellitus (T2DM) and coexisting non-alcoholic fatty liver disease (NAFLD), particularly in the elderly population. Nevertheless, in Vietnam, research data specifically addressing this group remain scarce.


Objective: To assess the risk of advanced liver fibrosis using the FIB-4 score and to identify associated factors in older adults with type 2 diabetes mellitus and NAFLD.


Methods: A cross-sectional study was conducted at the Endocrinology Outpatient Clinic of Thong Nhat Hospital from September 2024 to June 2025, involving patients aged ≥60 years with type 2 diabetes mellitus and coexisting NAFLD.


Results: Among 355 patients evaluated for the risk of advanced liver fibrosis, 73.24% were classified as low risk, 20.00% as indeterminate risk, and 6.76% as high risk. Multivariate logistic regression analysis identified chronic kidney disease (OR = 2.78; 95% CI: 1.27–6.10; p = 0.011) and white blood cell count (OR = 0.73; 95% CI: 0.62–0.84; p < 0.001) as independent factors associated with the risk of advanced liver fibrosis.


Conclusion: In older adults with type 2 diabetes mellitus and NAFLD, a considerable proportion were classified as having indeterminate or high risk of advanced liver fibrosis. Chronic kidney disease and white blood cell count were identified as independent factors closely associated with the risk of advanced liver fibrosis in this population.
