THE CLINICAL CHARACTERISTICS AND LESION MORPHOLOGY ON MAGNETIC RESONANCE IMAGING (MRI) AND SHOULDER ARTHROSCOPY IN PATIENTS WITH ROTATOR CUFF TEARS WHO UNDERWENT SURGICAL TREATMENT AT 108 MILITARY CENTRAL HOSPITAL

Abstract
Objective: To describe the clinical characteristics and lesion morphology on magnetic resonance imaging (MRI) and shoulder arthroscopy in patients with rotator cuff tears who underwent surgical treatment at 108 Military Central Hospital.
Material and methods: A retrospective cross-sectional descriptive study was conducted on 32 patients with rotator cuff tears who underwent surgical treatment from October 2024 to June 2025. Data were collected from medical records, clinical examinations, preoperative MRI findings, and operative reports.
Results: The mean age was 60.72 ± 10.32 years; 59.37% were aged ≥65 years, and 53% were female. Most patients had no history of shoulder trauma (84.37%) and had right-sided lesions (78%). Shoulder pain and weakness were present in all cases; mean VAS was 6.68 ± 1.65, and the Jobe test was positive in 100%. On MRI, isolated supraspinatus tear was the most common finding (65.62%), with grade I tendon retraction (78.12%) and Goutallier grade I fatty infiltration (68.75%) predominating. Mean tear size on arthroscopy was significantly greater than on MRI (3.34 ± 1.10 cm vs 3.12 ± 1.08 cm, p < 0.001). Arthroscopy detected more long head of biceps lesions than MRI, whereas labral lesions and subacromial space narrowing showed similar rates.
Conclusion: Rotator cuff tears were observed mainly in elderly patients, with shoulder pain and functional impairment being the predominant clinical manifestations. MRI is highly valuable for preoperative assessment, whereas arthroscopy allows more accurate determination of actual tear size and certain associated intra-articular lesions.
