ACUTE MYOCARDIAL INFARCTION IN A YOUNG ADULT WITH A HIGH POLYGENIC RISK SCORE FOR CORONARY ARTERY DISEASE: A CASE REPORT

Abstract
Background: Acute myocardial infarction (AMI) in young adults is increasingly recognized and is often related to a complex interplay between traditional cardiovascular risk factors and genetic predisposition. Polygenic risk scores for coronary artery disease (CAD) are emerging tools that quantify inherited risk and are particularly useful in young individuals, in whom conventional clinical risk scores may underestimate true risk.
Methods: Case report.
Results: A 34-year-old man was admitted with extensive anterior ST-segment elevation myocardial infarction complicated by cardiogenic shock and underwent percutaneous coronary intervention with stent implantation in the proximal left anterior descending artery. The patient had several traditional risk factors, including heavy smoking, mixed dyslipidemia, and obesity. Genetic testing did not identify pathogenic variants associated with familial hypercholesterolemia; however, the CAD polygenic risk score was high. After timely optimal medical therapy and percutaneous coronary intervention, the patient’s clinical condition stabilized and was discharged safely.
Conclusion: This case highlights the important role of polygenic inheritance in the pathogenesis of AMI in young adults. Integrating polygenic risk scores for CAD with clinical assessment may facilitate early identification of individuals at very high risk and support personalized secondary prevention strategies.
