CHARACTERISTICS OF RED BLOOD CELL DISTRIBUTION WIDTH IN PATIENTS WITH PULMONARY ARTERIAL HYPERTENSION IN VIETNAM NATIONAL HEART INSTITUTE, BACH MAI HOSPITAL

Abstract

Objective: Pulmonary arterial hypertension (PAH) causes chronic tissue hypoxia and systemic inflammation, which impair erythropoiesis (red blood cell production), leading to increased heterogeneity in red blood cell distribution width (elevated RDW-CV) and a poorer prognosis in heart failure. This study aimed to investigate RDW-CV (Red Cell Distribution Width–Coefficient of Variation)and its association with clinical, paraclinical, and hemodynamic parameters in patients with PAH.


Methods: This cross-sectional descriptive study included 119 patients with PAH who were treated at Vietnam National Heart Institute, Bach Mai Hospital.


Results: The patients had a mean age of 39.5 ± 15.9 years; 70.6% were female, and 63.0% had congenital heart disease. The mean RDW-CV was 15.1 ± 3.0%. RDW-CV showed: A positive correlation with mean pulmonary artery pressure (r = 0.230; p = 0.012); A positive correlation with pulmonary vascular resistance (r = 0.281; p = 0.002); A negative correlation with tricuspid annular plane systolic excursion (TAPSE) (r = −0.252; p = 0.006). Patients with right heart failure (35.3%) had significantly higher RDW-CV values than those without heart failure (16.2 ± 3.47% vs. 14.6 ± 2.56%; p = 0.004).


Conclusions: Our findings demonstrate that RDW is significantly associated with right heart failure in patients with pulmonary arterial hypertension, while also representing a cost-effective and readily accessible parameter for routine clinical practice.
