EVALUATION OF THE OUTCOMES OF MODIFIED FRONTALIS MUSCLE FLAP SURGERY IN THE TREATMENT OF SEVERE PTOSIS

Abstract
Objective: To evaluate the outcomes of modified frontalis muscle flap surgery in severe ptosis with poor levator function.
Subjects and methods: A retrospective descriptive study without a control group was conducted on 16 patients (20 eyes) with severe ptosis who underwent modified frontalis muscle flap surgery at Hanoi Medical University Hospital from February 2025 to August 2025. Outcome measures included MRD1, palpebral fissure height, lagophthalmos, aesthetic results, and complications before surgery, at 1 week, and at 6 months after surgery.
Results: MRD1 increased from 0.18 ± 0.29 mm preoperatively to 2.47 ± 1.11 mm at 1 week and 3.04 ± 0.62 mm at 6 months. Palpebral fissure height increased from 5.66 ± 1.36 mm to 8.08 ± 1.15 mm at 1 week and 8.65 ± 1.11 mm at 6 months. Lagophthalmos decreased from 3.98 ± 1.51 mm at 1 week to 1.99 ± 1.78 mm at 6 months. No corneal complications were observed, undercorrection occurred in 2/20 eyes and required reoperation.
Conclusion: Modified frontalis muscle flap surgery is an effective and safe method for severe ptosis, with marked improvement in MRD1 and palpebral fissure height and a gradual reduction in lagophthalmos over time.
