CONDYLAR POSITIONAL CHANGES AFTER BILATERAL SAGITTAL SPLIT RAMUS OSTEOTOMY COMBINED WITH LE FORT I OSTEOTOMY

Abstract
Objective: Evaluate short- and medium-term changes in condylar position using cone-beam computed tomography after mandibular advancement by bilateral sagittal split ramus osteotomy combined with Le Fort I osteotomy.
Methods: A descriptive retrospective study was conducted in 25 patients. CBCT data of 50 temporomandibular joints were obtained before surgery (T0), immediately after surgery (T1), 3 months after surgery (T2), and at the final follow-up after 12.1 ± 3.0 months (T3). The condyle-fossa relationship was assessed using the Pullinger and Hollender method.
Results: The ln(PS/AS) value changed significantly over time, with no significant difference between the right and left condyles. The condyles moved inferoposteriorly immediately after surgery and then anterosuperiorly at 3 months. The superimposed change indicated a posterosuperior condylar position compared with baseline, which remained relatively stable at 1-year follow-up.
Conclusion: Condylar positional changes occurred after BSSO combined with Le Fort I osteotomy. The condyles tended to return to a more concentric position by 3 months and remained stable during the 1-year follow-up.
