CHARACTERISTICS OF CENTRAL VENOUS CATHETER-RELATED THROMBOSIS IN THE PEDIATRIC INTENSIVE CARE UNIT

Abstract
Background: Venous thrombosis in children is frequently associated with the use of central venous catheters (CVC). A comprehensive understanding of the characteristics and associated factors of this complication is essential for optimizing prevention and management strategies within pediatric intensive care settings.
Objectives: To determine the incidence and associated factors of central venous catheter-related thrombosis (CRT) in pediatric patients at the Intensive Care Unit, Children's Hospital 1.
Methods: A longitudinal, descriptive – analytical study conducted at Children’s Hospital 1 from January 1, 2025, to July 31, 2025.
Results: A total of 90 patients met the inclusion criteria during the study period, with 115 CVCs inserted. The incidence of CVC-related thrombosis (CRT) was 24% (28/115), of which deep vein thrombosis accounted for 10.4%. Multivariable Cox regression analysis identified age and catheter insertion site as two independent factors associated with CRT development. Younger age was found to increase the risk of thrombosis (HR = 0.98; 95% CI: 0.97 – 0.99; p = 0.002). Compared to the femoral vein, CVC insertion at the internal jugular vein (HR = 0.09; 95% CI: 0.04 – 0.24; p < 0.001) and the subclavian vein (HR = 0.03; 95% CI: 0.01 – 0.28; p = 0.002) significantly reduced the risk of thrombosis formation.
Conclusion: The study enhances clinical awareness of CRT while identifying younger age and femoral vein insertion as two key factors associated with an increased risk of thrombus formation. Active screening with Doppler ultrasound is essential for early detection and timely management of this complication, particularly in high-risk groups.
