EVALUATION OF MANDIBULAR STABILITY AND FUNCTIONAL RECOVERY AFTER BILATERAL SAGITTAL SPLIT OSTEOTOMY COMBINED WITH GONIAL ANGLE REDUCTION IN SKELETAL CLASS III PATIENTS

Abstract
Objective: To evaluate mandibular skeletal stability, changes in the temporomandibular joint space, and recovery of occlusal function after bilateral sagittal split osteotomy (BSSO) combined with mandibular angle reduction in patients with skeletal Class III deformity.
Subjects and methods: A descriptive longitudinal follow-up study was conducted on 40 patients with skeletal Class III deformity due to mandibular prognathism who underwent BSSO combined with mandibular angle reduction at Van Hanh General Hospital from January 2021 to January 2023. Assessments were performed at T0 (preoperative), T1a (1 week), T1b (1 month), T2 (1 year), and T3 (3 years).
Results: Skeletal relapse from T1a to T3 was minimal, with point B increasing by +1.1 mm and pogonion by +1.3 mm. The vertical temporomandibular joint space changed after surgery and returned close to the baseline value after 3 years. Left–right occlusal force discrepancy decreased from 28.5% to 2.5%, while the number of occlusal contact teeth increased from 2 to 18 (p < 0.001). The total OQLQ score decreased from 65.0 to 7.5 after 3 years.
Conclusion: BSSO combined with mandibular angle reduction provides good skeletal stability, restores occlusal balance, and significantly improves quality of life after 3 years of follow-up in patients with skeletal Class III deformity.
