SINGLE-SITE LAPAROSCOPIC SURGERY: A NEW ADVANCE IN MINIMALLY INVASIVE SURGERY FOR THE TREATMENT OF OVARIAN TUMORS AND BROAD LIGAMENT CYSTS IN CHILDREN

Abstract
Introduction: Conventional laparoscopy using 3 or 4 ports is the traditional method for treating ovarian tumors and broad ligament cysts in children. With the evolution of minimally invasive surgery, Single-Site Laparoscopic Surgery (SSLS) has emerged as a significant advancement. However, reports on the application of this technique for ovarian tumors and broad ligament cysts in pediatric patients remain limited. This study aims to describe the initial experience of implementing SSLS for these conditions and to analyze the advantages and disadvantages of the method.
Patients and Methods: This is a retrospective case series study of all pediatric patient underwent SSLS for ovary-sparing tumor excision and broad ligament cyst resection at Children's Hospital 1 between January 2022 and January 2026.
Results: A total of 25 female pediatric patients diagnosed with ovarian tumors and broad ligament cysts underwent single-site surgery. The mean age at surgery was 11.36 years (range: 7–15 years). The mean operative time was 73.4 minutes (range: 40–125 minutes). There were no cases requiring intraoperative blood transfusion, additional trocar placement, conversion to open surgery, or postoperative drainage. All patients were extubated immediately after surgery. The mean postoperative hospital stay was 1.92 days (range: 1–4 days), with no postoperative complications recorded. Histopathological results included: 17 mature teratomas, 1 dermoid cyst, and 7 paratubal cysts. The mean follow-up period was 21.52 months (range: 1–42 months), with no signs of recurrence.
Conclusion: Single-site laparoscopic surgery is a safe and effective treatment modality and an excellent alternative to conventional laparoscopy for adnexal pathologies. Utilizing a single umbilical trocar site—which can be slightly extended—instead of three or four ports facilitates the excision and removal of ovarian/adnexal masses while ensuring superior aesthetic outcomes with virtually no visible surgical scarring.
