SURGICAL RESULTS OF MALIGNANT DUODENAL LESIONS UPPER AND LOWER AMPULLA OF VATER

Abstract
Results: There were 34 patients, men 20 patients, female 14 patients; mean age 64±8,48; Clinical features: Abdominal pain 91,2%, vomiting 58,8%, melena 8,8%, hematemesis 5,9%, obstruction 5,9%, gastric outlet obstruction 44,1%... There were 6 groups of duodenal malignant lesions. Surgical Procedure:
Radical surgery in 86,7%: Duodenal segmentectomy with antrectomy and duodenostomy or wedge resection with gastrojejunostomy or duodenostomy. Paliative surgery in 13,3%
No death, Complication : Anastomotic duodeno-duodenal fistulas in 8,8%.
Conclusion: There were 6 groups of malignant duodenal lesions: Duodenal GIST; Gastriccancer invaded duodenum; Colonic cancer invaded duodenum; Duodenal AC; lymphoma of duodenum; Leiomyoma of duodenum.
Surgical procedure: Duodenal segmentectomy with antrectomy and duodenostomy could have favorable results with low complication. Pancreato duodenectomy could be done if the malignant lesions invading the ampulla of Vater or duodenal layer and pancreatic head.
