Abstract

Objective: To evaluate the surgical outcomes of closed Monteggia fractures in adults treated with ulnar plate fixation at Viet Tiep Friendship Hospital.

Material and methods: A cross-sectional descriptive study combining retrospective and prospective data was conducted on 31 adult patients with closed Monteggia fractures who underwent surgical treatment with ulnar plate fixation at Viet Tiep Friendship Hospital from January 2020 to June 2024. Treatment outcomes were evaluated based on radiographic bone union and functional recovery of the elbow using the Mayo Elbow Performance Score (MEPS).

Results: The mean age of patients was 36.68 ± 12.68 years, with males accounting for 58.1% of cases. The most common injury mechanism was direct trauma (71%). According to the Bado classification, type I accounted for 61.3%. After surgery, all radial heads were successfully reduced to their anatomical position, and no cases of recurrent radial head dislocation were observed. Most patients achieved good bone union within 6–12 months, and functional evaluation using the MEPS demonstrated good to excellent outcomes in the majority of cases.

Conclusion: Surgical treatment of Monteggia fractures in adults using ulnar plate fixation is a safe and effective method, providing good anatomical restoration and favorable functional recovery of the elbow joint.
