CLINICAL AND PARACLINICAL CHARACTERISTICS AND INITIAL TREATMENT OUTCOMES OF STAGE IV THYMIC CARCINOMA WITH FIRST-LINE PACLITAXEL-CARBOPLATIN REGIMEN

Abstract
Objectives: To describe the clinical and paraclinical characteristics, objective response rate, and adverse effects of stage IV thymic carcinoma treated with first-line Paclitaxel–Carboplatin (PC) regimen.
Subjects and Methods: A retrospective study of 59 patients with stage IV thymic carcinoma (WHO 2021; TNM version 9), Eastern Cooperative Oncology Group (ECOG) performance status 0–1, treated with first-line PC regimen from January 2016 to December 2025 at K Hospital and National Lung Hospital. Treatment response was assessed according to RECIST 1.1 and toxicity according to CTCAE v5.0.
Results: Mean age 52.7 ± 14.8 years; male 67.8%; ECOG 0: 54.2%. Chest pain (50.8%), cough (35.6%), and fatigue (33.9%) were the most common symptoms. Squamous cell carcinoma accounted for 66.1%; stage IVB 64.4%; pleural metastasis 47.5%. No complete response (CR) was achieved; partial response (PR) 45.8%, stable disease (SD) 39%, and progressive disease (PD) 15.2%. Objective response rate (ORR) was 45.8% and disease control rate (DCR) was 84.8%. Common toxicities included anemia (62.7%), peripheral neuropathy (52.5%), and leukopenia (45.8%), predominantly grade 1–2; no grade 4 toxicity and no treatment-related mortality were recorded.
Conclusions: The Paclitaxel–Carboplatin regimen achieved a moderate response rate with an acceptable safety profile, consistent with the NCCN v2.2026 priority recommendation for stage IV thymic carcinoma.
