OUTCOMES OF ALTEPLASE TREATMENT AT DOSES OF 0.6 MG/KG AND 0.9 MG/KG IN ACUTE ISCHEMIC STROKE PATIENTS AT HUU NGHI HOSPITAL

Abstract
Objective: To describe the treatment outcomes of alteplase at doses of 0.6 mg/kg and 0.9 mg/kg in patients with acute ischemic stroke at Huu Nghi Hospital.
Subjects: Patients with acute ischemic stroke who underwent intravenous thrombolysis at Huu Nghi Hospital from January 2020 to December 2025.
Methodology: Retrospective descriptive study
Results: The 0.6 mg/kg dose group predominantly consisted of patients aged ≥ 80 years (p < 0.05). Both doses significantly reduced NIHSS scores after 24 hours (p < 0.01. After 3 months, the 0,9 mg/kg dose group demonstrated a higher rate of good functional recovery (mRS 0 – 1) compared to the 0,6 mg/kg group (56,4% vs 40,4%, p < 0,05). Although the 0,9 mg/kg dose group had a higher overall rate of intracranial hemorrhage (p < 0,05), the incidence of symptomatic intracranial hemorrhage was comparable between the two groups (p > 0,05).
Conclusions: The 0,9 mg/kg tPA dose achieved better 3-month functional recovery than the 0,6 mg/kg dose (p < 0,05) but was associated with a higher overall rate of intracranial hemorrhage (p < 0,05); however, the rates of symptomatic intracranial hemorrhage were similar between the two groups (p > 0,05).
