RADIATION DOSE AND ASSOCIATED FACTORS IN PATIENTS UNDERGOING CORONARY ANGIOGRAPHY AND PERCUTANEOUS CORONARY INTERVENTION ON THE AZURION 5C20 DSA SYSTEM AT HUNG VUONG GIA LAI HOSPITAL
Main Article Content
Abstract
Background: Interventional cardiology involves the highest radiation doses among diagnostic X-ray procedures. Empirical assessment of radiation exposure levels on the new-generation Azurion 5C20 DSA system equipped with ClarityIQ technology is crucial for optimizing radiation safety according to the ALARA principle.
Objectives: (1) To describe five core radiation dose metrics (total DAP, DAPcine, DAPfluo, cumulative Air Kerma Kar, and fluoroscopy time FT) in patients undergoing coronary angiography (CAG) and percutaneous coronary intervention (PCI); (2) To analyze related factors (BMI, procedure type, FT) and establish a multivariable regression model.
Methods: A retrospective cross-sectional study was conducted on 69 patients (CAG: 32; CAG+PCI: 37) from June to December 2024 at Hung Vuong Gia Lai Hospital. Data were extracted from EHC and ClarityIQ software. Statistical analysis was performed using SPSS 26.0 (Mann-Whitney U test, Spearman correlation, and log-transformed multivariable linear regression).
Results: The PCI group received a radiation burden 2.5-2.7 times higher than the CAG group (p<0.001): median total DAP for PCI reached 78.6 Gy.cm² vs. CAG 31.4 Gy.cm²; Air Kerma Kar for PCI was 1260 mGy vs. CAG 490 mGy; FT for PCI was 12.2 minutes vs. CAG 4.5 minutes. Univariate correlation revealed that FT had a very strong positive correlation with total DAP (r=0.815; p<0.001) and Air Kerma (r=0.852; p<0.001). No cases received a dose of Kar≥5.0 Gy. The multivariable regression model explained 76.8% of the variation in DAP (R²=0.768) and 81.2% of the variation in Kar (R²=0.812).
Conclusion: The Azurion 5C20 DSA system ensures good radiation safety. Establishing local Diagnostic Reference Levels (Local DRLs) and strictly controlling fluoroscopy time are necessary to minimize radiation exposure.
