INHALER ADHERENCE AND RELATED FACTORS IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE AT TAM ANH GENERAL HOSPITAL HANOI 2025

Abstract
Objectives: To evaluate the status of inhaler adherence and identify associated factors among patients with Chronic Obstructive Pulmonary Disease (COPD).
Methods: A cross-sectional descriptive study was conducted on 74 COPD patients from March 2025 to January 2026. Data collection tools included clinical characteristics (mMRC and CAT scales), the Test of Adherence to Inhalers (TAI-10), and the Beliefs about Medicines Questionnaire (BMQ-COPD). Data were analyzed using Chi-square tests, Fisher’s exact tests, and multivariable logistic regression.
Results: The mean age of the participants was 68.4 ± 8.0 years, with a male predominance of 81.1%. The rate of good inhaler adherence was 58.1%. Among the two non - adherence phenotypes, the non-intentional phenotype was most prevalent 40.5%, primarily characterized by forgetfulness. Multivariable analysis identified three independent predictors of good adherence: mild dyspnea (mMRC < 2; AOR = 3.94; p = 0.042), higher ‘Necessity’ scores (AOR = 1.23; p = 0.047), and lower ‘Concerns’ regarding medication (AOR = 0.79; p = 0.023). Age, gender, and CAT scores showed no statistically significant associations (p > 0.05).
Conclusion: More than half of the surveyed patients achieved good inhaler adherence. Medication beliefs and dyspnea severity are the primary factors influencing adherence behavior. Intervention strategies should prioritize counseling to alleviate anxiety regarding side effects and strengthening treatment beliefs, particularly in patients with high symptom burden.
