GASTRIC MEDULLARY CARCINOMA - DIAGNOSTIC PITFALL IN PATHOLOGY: A CASE REPORT

Abstract

Gastric medullary carcinoma is a rare histological subtype of gastric adenocarcinoma which is usually associated with Epstein-Barr virus and could be misdiagnosed. We reported a case of a 60-year-old man having a gastric ulcer with a previous diagnosis of chronic active ulcerative gastritis from another facility. Malignancy could not be excluded on re-biopsied specimen. Endoscopic submucosal dissection was performed. The histopathological examination revealed a malignant tumor with poorly differentiated morphology and lymphoid stroma. Ancillary studies revealed tumor cells were positive for epithelial markers and EBER in situ hybridization. A diagnostic of gastric medullary carcinoma was established. The patient had undergone partial gastrectomy with lymphadenectomy. 18 months after surgery, neither locoregional recurrence nor distant metastasis was found. This case highlights that gastric medullary carcinoma could mimic other entities and the definitive diagnosis requires combination of routine histopathological examination and ancillary studies, especially EBER in situ hybridization.
