Abstract

Background: Healthcare-associated infections are a major global concern, and surgical hand hygiene (SHH) among surgeons plays a critical role in preventing surgical site infections.


Objective: To determine the compliance rate with SHH procedures among surgeons and associated factors at Tam Anh General Hospital, Ho Chi Minh City, in 2025.


Methods: A descriptive cross-sectional study was conducted from April to December 2025 using direct observation of SHH events among surgeons with standardized checklists. Data were analyzed using R software, and multivariable analysis was performed with generalized estimating equations (GEE).


Results: The overall compliance rate with SHH procedures was 70.2%, with compliance to technical steps and recommended duration of 94.9% and 72.8%, respectively. Compliance differed significantly across surgical specialties; compared with orthopedic surgeons, breast surgeons showed higher compliance (OR = 2.58; 95% CI: 1.07–6.21), whereas otorhinolaryngology surgeons had lower compliance (OR = 0.16; 95% CI: 0.06–0.44). Compliance was also significantly lower during off-hours compared with regular working hours (OR = 0.65; 95% CI: 0.49–0.85). No significant associations were observed between compliance and demographic or professional characteristics, including sex, age, qualification, and years of experience.


Conclusion: Surgeons’ compliance with SHH procedures was moderate. System-level factors, particularly surgical specialty and timing of surgery, were significantly associated with compliance.
