Abstract

Piriformis syndrome (PS) is a rare cause of buttock pain and sciatica, most commonly resulting from trauma, inflammation, muscle spasm, or degenerative changes. However, congenital anatomical variants represent an uncommon etiology. We report the case of a 59-year-old woman diagnosed with piriformis syndrome caused by a type B anatomical variant according to the Beaton and Anson classification, in which the common fibular divition of the sciatic nerve passes between the two bundles of the piriformis muscle. The patient achieved initial clinical improvement with conservative management consiting of ultrasound-guided corticosteroid injection combined with piriformis stretching exercises. This case report aims to increase awareness among neurologists and musculoskeletal physicians of piriformis syndrome, particularly in patients presenting with atypical chronic sciatica.
