A PRELIMINARY EVALUATION OF AIRWAY CHANGES FOLLOWING ORTHODONTIC TREATMENT WITH EXTRACTION OF FOUR PREMOLARS IN PATIENTS WITH BIMAXILLARY PROTRUSION

Abstract
Objective: To evaluate changes in upper airway dimensions on lateral cephalometric radiographs before and after orthodontic treatment involving the extraction of four premolars in patients with bimaxillary protrusion.
Materials and Methods: This retrospective study included 32 lateral cephalometric radiographs obtained from 16 patients with bimaxillary protrusion and Class I malocclusion who underwent orthodontic treatment with extraction of four permanent premolars using maximum anchorage with mini-implants. The patients were treated at School of Dentistry, Hanoi Medical University, and the Department of Orthodontics, National Hospital of Odonto-Stomatology, Hanoi. Airway dimensions at the nasopharyngeal, oropharyngeal, and hypopharyngeal levels were measured on lateral cephalometric radiographs obtained before and after treatment.
Results: The dimensions of the oropharyngeal airway (SPAS, MAS, and IAS) and hypopharyngeal airway (LPW-Eb) decreased significantly after treatment (p < 0.01), whereas no significant changes were observed in the nasopharyngeal airway. Specifically, SPAS decreased by 1.27 ± 1.24 mm, MAS by 0.87 ± 1.09 mm, IAS by 1.52 ± 1.32 mm, and LPW-Eb by 1.88 ± 2.09 mm.
Conclusions: Orthodontic treatment with extraction of four premolars combined with maximum anchorage using mini-implants resulted in a significant reduction in the dimensions of the oropharyngeal and hypopharyngeal airways on lateral cephalometric radiographs in patients with bimaxillary protrusion and Class I malocclusion.
