Abstract

Objective: To describe abdominal ultrasound, radiographic, and CT findings in patients with hollow viscus perforation at Gia Lam General Hospital.


Methods: A cross-sectional study of 40 surgically confirmed patients from January 2022 to July 2025. Data were analyzed using SPSS 20.0.


Results: Male/female ratio was 1.8/1; mean age 55 ± 21 years (75% aged ≥ 40). Perforation sites: anterior gastric wall (85%), pylorus-duodenum (7.5%), posterior gastric wall (5%), fixed duodenum (2.5%). Peptic ulcer disease was the main etiology (87.5%). Ultrasound: Free fluid (82.5%), prehepatic air (62.5%), concurrent fluid/air (60%), fat stranding (22.5%), bowel thickening (7.5%), ectopic air (5%), bowel wall discontinuity (2.5%). Radiography: Overall pneumoperitoneum rate was 80% (bilateral: 45%, unilateral: 32.5%, perinephric air: 2.5%). 72.5% were detected initially, 7.5% on repeat views, and 20% remained negative. Non-contrast CT: Pneumoperitoneum was 100% (subdiaphragmatic/prehepatic: 100%, periportal: 67.5%, interloop: 65%, pelvis: 60%, retroperitoneal: 2.5%). Associated signs: surrounding fat stranding (87.5%), adjacent air bubbles (85%), bowel thickening (75%), free fluid (72.5%), bowel wall discontinuity (30%), fluid collection (10%).


Conclusion: Ultrasound and radiography are effective first-line screening tools. Non-contrast CT is the most valuable modality at primary care, yielding 100% sensitivity for pneumoperitoneum and accurately localizing the perforation site.
