Abstract

Objective: To describe the clinical and radiographic characteristics before surgery and evaluate changes in the lower limb mechanical axis after total knee arthroplasty in patients with knee osteoarthritis and axial deformity greater than 5°.


Subjects and Methods: A descriptive, retrospective and prospective longitudinal study without a control group was conducted on 40 patients with knee osteoarthritis and axial deformity >5° who underwent total knee arthroplasty at Military Central Hospital 108 from April 2025 to January 2026.


Results: In 40 patients with knee osteoarthritis and axial deformity >5°, females accounted for 87.5%, the 60–69-year age group was the most prevalent (57.5%), and 70% of patients had grade IV osteoarthritis according to the Kellgren–Lawrence classification. After surgery, the mean mechanical axis angle decreased from 9.68 ± 3.84° to 2.11 ± 0.91°, with a mean correction of 7.57 (p < 0.001). The mean KS score increased from 44.1 ± 7.6 to 84.2 ± 2.1 points, while the KFS score increased from 44.8 ± 6.2 to 84.9 ± 2.0 points (p < 0.001).


Conclusion: Total knee arthroplasty significantly corrected mechanical axis deformity and improved knee function in patients with advanced knee osteoarthritis. The preliminary 6-month results demonstrated good effectiveness in restoring lower limb alignment and knee function.
