OUTCOMES OF THE MANAGEMENT OF UTERINE FIBROIDS DURING CESAREAN DELIVERY AT THANH HOA OBSTETRICS AND GYNECOLOGY HOSPITAL

Abstract
Objective: To evaluate the outcomes of uterine fibroid management during cesarean delivery at Thanh Hoa Obstetrics and Gynecology Hospital from January 2023 to August 2025.
Subjects and Methods: A retrospective descriptive study was conducted on 165 pregnant women with uterine fibroids who underwent management during cesarean delivery at Thanh Hoa Obstetrics and Gynecology Hospital between January 2023 and August 2025. The study included women with viable pregnancies of at least 22 weeks’ gestation who were diagnosed with uterine fibroids before pregnancy, during pregnancy, or at the time of cesarean delivery.
Results: The mean maternal age was 33.47 ± 5.36 years, with 63.7% of women younger than 35 years. Myomectomy was the most commonly performed management approach, accounting for 72 cases (43.7%), followed by no intervention on the fibroid in 54 cases (32.7%) and hysterectomy in 39 cases (23.6%). The management approach was significantly associated with fibroid size (p = 0.005), number of fibroids (p = 0.002), and fibroid location (p < 0.001). The rate of hysterectomy was higher among women with fibroids >10 cm and those with three or more fibroids. No significant association was observed between fibroid location and intraoperative hemorrhage (p = 0.078). The postoperative blood transfusion rate was 5.5%, and two patients required reoperation with hysterectomy due to postpartum hemorrhage caused by uterine atony.
Conclusion: Myomectomy was the most frequently performed management approach during cesarean delivery in women with uterine fibroids. The choice of management was associated with fibroid size, number, and location. Appropriate management of uterine fibroids during cesarean delivery yielded favorable outcomes when performed in selected cases.
