Abstract

Objective: To evaluate the predictive value of the DECAF score for adverse in-hospital outcomes in patients hospitalized with acute exacerbations of chronic obstructive pulmonary disease (AECOPD).

Methods: This prospective observational study included 108 patients hospitalized with AECOPD at Long An General Hospital between January 2025 and May 2026. The DECAF score was calculated at hospital admission.

Results: The mean age was 69.4 ± 9.8 years, and 75.9% of the patients were male. The median DECAF score was 2 (IQR, 1–3), with 35 patients (32.4%) having a DECAF score ≥3. Adverse in-hospital outcomes occurred in 30 patients (27.8%), and 10 patients (9.3%) died during hospitalization. The proportion of patients with adverse outcomes increased progressively with increasing DECAF scores (p for trend <0.001). The DECAF score was significantly higher in patients with adverse outcomes than in those without adverse outcomes [3 (IQR, 2–4) vs. 1 (IQR, 1–2), p<0.001]. The area under the ROC curve for predicting adverse in-hospital outcomes was 0.842 (95% CI, 0.763–0.921; p<0.001). At a cutoff of ≥3 points, the DECAF score yielded a sensitivity of 70.0% and a specificity of 82.1%.

Conclusion: Higher DECAF scores were associated with a greater frequency of adverse in-hospital outcomes in patients hospitalized with AECOPD. The DECAF score showed good discrimination for adverse outcomes and may complement routine clinical and laboratory assessment for early risk stratification.
