Abstract

Objective: To describe maternal serum PAPP-A and PLGF concentrations in the first trimester of pregnancy (11+0-13+6 weeks) at Hai Phong Obstetrics Hospital.

Methods: A retrospective descriptive study was conducted on 1,297 singleton pregnancies screened for PAPP-A and PLGF at 11-13+6 weeks, with complete pregnancy outcome data, at Hai Phong Obstetrics Hospital during 2024-2025. Absolute concentrations and gestational-age-adjusted multiples of the median (MoM) were described by gestational age at sampling, maternal age group, and conception type. Data were analyzed with Stata 17.0, using the Mann-Whitney test for between-group comparisons.

Results: Mean maternal age was 29.5 ± 5.2 years; 17.8% were ≥35 years. Mean PAPP-A was 4.18 ± 2.86 mIU/mL (median MoM 0.86; IQR 0.62-1.18); mean PLGF was 59.64 ± 27.13 pg/mL (median MoM 0.91; IQR 0.72-1.18). Both markers showed right-skewed distributions. Absolute concentrations increased with gestational age (PAPP-A: 2.92 → 4.13 → 5.74 mIU/mL; PLGF: 51.22 → 59.49 → 68.42 pg/mL from 11 to 13 weeks), while MoM values remained stable around 1 across all three gestational weeks. No significant difference in PAPP-A or PLGF MoM was found between maternal age groups <35 and ≥35 years (p=0.157 and p=0.665). MoM values tended to be higher in assisted-reproduction pregnancies (IUI/IVF) than in natural conceptions, although the intervention subgroup was small.

Conclusion: Maternal serum PAPP-A and PLGF concentrations in the first trimester at Hai Phong Obstetrics Hospital increase with gestational age, consistent with published data. MoM values were stable around 1 and did not differ by maternal age, indicating that the gestational-age adjustment currently applied is appropriate for this population and provides a reliable basis for subsequent predictive analyses of pregnancy outcomes.
