Abstract

Objective: To evaluate anatomical, audiological, and composite outcomes of endoscopic myringoplasty in patients with non-cholesteatomatous chronic otitis media.

Subjects and methods: A prospective descriptive case series included 96 patients who underwent endoscopic myringoplasty at Thanh Hoa Provincial General Hospital between July 2025 and July 2026. Air-conduction pure-tone average (PTA) was calculated at 500, 1,000, 2,000, and 4,000 Hz. Clinical symptoms, endoscopic findings, and audiometry were assessed before surgery and at 3 months. Overall outcome was categorized as good, fair, or poor according to complications, PTA improvement, graft healing, tympanic scarring, reperforation, and otorrhea.

Results: Mean age was 46.87 ± 13.82 years, and 85.4% were female. Disease duration exceeded 5 years in 69.8%. Central and medium-sized perforations accounted for 69.8% and 56.3%, respectively. The overlay technique was used in 85.4%, and temporalis fascia in 98.0%; no intraoperative complication was recorded. At 3 months, graft healing was achieved in 92/96 patients (95.8%), all ears were dry, and reperforation occurred in 4 cases (4.2%). Mean PTA improved from 42.34 ± 12.84 dB to 25.61 ± 12.49 dB, with a mean gain of 16.73 ± 8.86 dB (p<0.001). Normal hearing or mild hearing loss was observed in 78.1%. Overall outcomes were good in 70 patients (72.9%), fair in 22 (22.9%), and poor in 4 (4.2%).

Conclusion: Endoscopic myringoplasty achieved a high short-term graft-healing rate and significant hearing improvement. At 3 months, 95.8% of patients had at least a fair composite outcome. Longer follow-up and analytical studies of failure predictors are warranted.
