OUTCOMES OF ARTHROSCOPIC SURGERY FOR ANTERIOR SHOULDER INSTABILITY WITH HILL-SACHS LESIONS
Main Article Content
Abstract
Objective: To evaluate 12-month outcomes of arthroscopic surgery for anterior shoulder instability with Hill-Sachs lesions.
Subjects and methods: This was a prospective interventional longitudinal study without a control group. The analysis included 63 patients with recurrent traumatic anterior shoulder instability and Hill-Sachs lesions who underwent arthroscopic surgery at Military Hospital 175 during the period from August 2022 to December 2025 and had at least 12 months of follow-up data. Outcomes included Rowe, ASES and UCLA scores, pain and instability visual analogue scales (VAS), shoulder range of motion, recurrent instability and complications.
Results: Isolated Bankart repair was performed in 39/63 patients (61.9%) and Bankart repair with Remplissage in 24/63 (38.1%). The mean Rowe score increased from 30.8 ± 8.3 preoperatively to 89.8 ± 8.2 at 12 months; ASES from 64.4 ± 9.4 to 91.0 ± 6.1; and UCLA from 21.6 ± 3.2 to 31.2 ± 2.2 (all p < 0.001). Pain VAS decreased from 4.3 ± 1.5 to 0.8 ± 0.7 and instability VAS from 7.4 ± 1.1 to 0.6 ± 0.9 (p < 0.001). At 12 months, 82.5% of patients were completely stable; complete redislocation occurred in 7.9%, subluxation in 4.8% and residual subjective instability in 4.8%. Good or excellent Rowe outcomes were achieved in 90.5%.
Conclusion: At 12 months, functional scores, pain and subjective instability improved significantly from baseline, with a high proportion of good-to-excellent Rowe outcomes. These findings indicate favorable short-term outcomes of arthroscopic Bankart repair, with or without Remplissage as clinically indicated, in this cohort.
