MIDDLE COLIC ARTERY ORIGINATING DIRECTLY FROM AORTA AND ABSENCE OF LEFT COLIC ARTERY: SERIES CASES REPORT AND LITERATURE REVIEW
Main Article Content
Abstract
Study aim: Evaluating the origin and course of Middle Colic Artery Originating Directly from Aorta and Absence of Left Colic Artery and Literature Review.
Result:
Male patient, 56 years old, got 1/3 left transverse colon cancer. Intra operating Finding: Middle Colic Artery Originating Directly from Aorta Proximal to superior mesenteric artery and divided into right branch that connect to ascending branch of right colic artery and left branch that annastomose with ascending branch of left colic artery.
Surgical Procedure: right extended hemicolectomy. Ligation of right branch of aberrant middle colic artery and side to side ileo- colono anastomosis.
Female patient, 68 years old, subobstruction for middle transverse colon cancer and rectal cancer. Intraoperation finding: absence of left colon artery (or acessory colonic artery measuring about 1mm) originating from inferior mesenteric artery. Surgical procedure: Transverse segmentectomy with middle colic artery ligation; end to end anastomosis. Protectomy with inferior mesenteric artery ligation, ultralow anastomosis.
Conclusion:
Middle Colic Artery Originating Directly from Aorta is rare aberrant Middle Colic Artery. Given the unusual origin and course, this artery is prone to iatrogenic injury and ligation leading to colon ischemia and anastomotic fistulas.
Absence of left colic artery is rare variant anatomy which leading to ischemia and anastomotic fistulas and colic necrosis due to middle colic artery ligation and inferior mesenteric artery ligation due to coexistence of 2 colic cancer.
