Abstract

Objective: To evaluate the outcomes of deep inferior epigastric perforator (DIEP) flap reconstruction following breast cancer surgery at Military Hospital 175.

Methods: A prospective observational case-series study was conducted in 37 patients who underwent breast reconstruction using deep inferior epigastric perforator (DIEP) flaps, corresponding to 40 flaps. Three patients underwent bilateral breast reconstruction at Military Hospital 175 between June 2023 and December 2025.

Results: The mean patient age was 46.8 ± 8.7 years, with a mean body mass index (BMI) of 23.8 ± 2.8 kg/m². Immediate breast reconstruction accounted for 62.2% of cases, while 27.0% of patients had received radiotherapy before reconstruction. Among the 40 flaps, one perforator was used in 60.0% and two perforators in 35.0%. The mean operative time was 482.6 ± 76.4 minutes. Complete flap survival was achieved in 95.0% of cases, with no total flap loss. No postoperative complications were observed in 67.6% of patients. The most common complications were breast seroma (10.8%), reoperation (10.8%), and abdominal seroma (8.1%). Aesthetic outcomes were rated as excellent or good in 81.1% of patients, and 81.1% were satisfied or very satisfied with the reconstruction.

Conclusion: DIEP flap breast reconstruction achieved a 95.0% complete flap survival rate, with no cases of total flap loss; 81.1% of patients had good or excellent aesthetic outcomes and were satisfied or very satisfied with the reconstruction.
