ASSOCIATION BETWEEN EARLY COMPLICATIONS AFTER CRANIOPLASTY IN PATIENTS WITH SKULL DEFECT FOLLOWING DECOMPRESSIVE CRANIECTOMY AND PREOPERATIVE PROGNOSTIC NUTRITIONAL INDEX

Abstract
Objective: To evaluate the association between the early complications within 30 days after cranioplasty in patients with skull defect following decompressive craniectomy and preoperative prognostic nutritional index (PNI).
Subjects and methods: A retrospective descriptive study of 70 patients with skull defect after decompressive craniectomy who underwent cranioplasty at Military Hospital 103 from October 2023 to February 2025. PNI was calculated as albumin (g/L) plus 5 times the absolute lymphocyte count (10⁹/L). Complications within 30 days were recorded from medical records. A multivariable logistic regression model adjusted for age was used.
Results: Mean age was 47.2 ± 13.6 years; 80.0% were male. Fourteen of 70 patients (20.0%) experienced at least one complication, most commonly surgical site infection (7.1%). The complication group had a lower PNI (42.7 ± 5.6 vs 49.0 ± 4.9; p=0.001) and lower albumin (p=0.003). After adjustment for age, each 1-point increase in PNI was associated with lower odds of complications (aOR 0.792; 95% CI: 0.676–0.927; p=0.004).
Conclusion: Preoperative PNI was associated with early complications after cranioplasty after adjustment for age and may provide additional information for preoperative risk assessment.
