Abstract

Objectives: To determine the incidence and identify factors associated with adverse reactions in plateletpheresis donors at the Blood Transfusion and Hematology Hospital (BTHH).

Study design: A cross-sectional descriptive study. Adverse reactions were classified according to ISBT/AABB/IHN 2014 standards; univariate and multivariate logistic regression analyses were used to identify risk factors.

Setting and time: Donor Selection Department, BTHH, from November 2025 to April 2026.

Subjects: 1,495 plateletpheresis donors at the Donor Selection Department, BTHH, during the study period.

Results: The overall incidence of adverse reactions was 6.7% (100/1,495), predominantly mild to moderate; citrate-related reactions were the most common (54%), followed by systemic reactions (26%) and local reactions (20%). Multivariate analysis identified five independent risk factors for overall adverse reactions: female sex (OR=2.41), age ≤25 (OR=2.24), first-time donors (OR=2.43), BMI<23 (OR=1.71), and prolonged apheresis duration (OR=2.41), with p<0.05.

Conclusions: Plateletpheresis donation at BTHH was highly safe, with citrate reactions being the most common. Female sex, age ≤25, first-time donation, BMI<23 and apheresis duration >100 minutes increased the odds of adverse reactions by 1.71 to 2.43 times; apheresis duration >100 minutes increased the odds of citrate reactions by 4.75 times.
