DIAGNOSTIC VALUE OF FLOW CYTOMETRY IN HEMATOLOGIC DISEASES: INITIAL IMPLEMENTATION EXPERIENCE AT DA NANG HOSPITAL
Main Article Content
Abstract
Background: Flow cytometry (FC) is an essential tool in the diagnosis and classification of hematologic diseases through immunophenotypic analysis. Since July 2024, FC has been implemented in the Hematology Department of Da Nang Hospital (DNH). This study aimed to evaluate the concordance between FC and bone marrow aspiration (BMA) and to identify the additional diagnostic value of FC.
Methods: A retrospective descriptive case-series study was conducted on 284 patients who underwent both FC and BMA at DNH between July 2024 and December 2025. Results of both methods were compared with the final diagnosis established according to the WHO 2022 classification. Agreement was evaluated using the Kappa coefficient. The added value of FC was determined based on its ability to confirm or modify the diagnosis compared with BMA alone. Factors associated with discordance were analyzed using logistic regression.
Results: FC and BMA were concordant in 85.9%, with a Kappa coefficient of 0.802 (95% CI: 0.744-0.854). FC contributed to a change in diagnosis in 7.8% of cases compared with BMA alone. Myeloid disorders were strongly associated with FC discordance relative to the final diagnosis compared with BMA (OR = 50.67; 95% CI: 7.51–341.74; p < 0.001).
Conclusion: FC demonstrates a high level of agreement with BMA and provides meaningful diagnostic value in hematologic diseases. Its diagnostic performance was more limited in myelodysplastic and myeloproliferative neoplasms. The integration of FC with BMA may improve diagnostic accuracy in routine clinical practice.
