Abstract

Objective: To evaluate selected quality indicators and the efficacy of the leukoreduction process for red blood cell (RBC) units prepared from 350-mL whole blood and leukoreduced within 72 hours after collection at Cho Ray Blood Transfusion Center.

Materials and Methods: A descriptive cross-sectional study was conducted on 60 units of 350-mL whole blood selected by convenience sampling from January 26 to April 15, 2026. The units were followed from whole blood through preparation of pre-leukoreduction and post-leukoreduction RBC units using LRF-BB filters (Tianhe, China). Volume, hematocrit, hemoglobin content, RBC count, and residual white blood cell (WBC) count were assessed according to Vietnamese Ministry of Health and European standards. The leukocyte removal efficiency and red blood cell recovery rate were calculated for each unit using the corresponding formulas. The mean of the rates calculated for the 60 units was then used to report the overall results.

Results: Among 60 units of 350-mL whole blood, 100% of leukoreduced red blood cell units met the applicable quality criteria. The median residual leukocyte count was 0.02 ×10⁶/unit (IQR: 0.002–0.05), with a median leukocyte removal efficiency of 99.9989% (IQR: 99.9976–99.9999) and a median red blood cell recovery of 87.64% (IQR: 85.93–88.46). Compared with pre-leukoreduction, volume, red blood cell count, hemoglobin, and leukocyte count decreased significantly (p<0.001), while hematocrit showed no significant change (p=0.137).

Conclusion: Red blood cell units prepared from 350-mL whole blood and leukoreduced within 72 hours of collection at Cho Ray Blood Transfusion Center met the applicable quality criteria for volume, hematocrit, hemoglobin, and residual leukocyte count, with a 100% compliance rate (60/60). The leukoreduction process achieved high leukocyte removal efficiency and a red blood cell recovery rate comparable to those reported in previous studies.
